How it works
Pay once a year at the office, and your preventive care for the next twelve months is covered. Everything else, from fillings to implants, is 10% off our standard fee. Family members can each choose the plan that fits them.
The fine print
Our membership plan is not insurance and is not a qualified health plan under the Affordable Care Act. Payment for treatment not included in the plan is due at the time of service. Plan benefits cannot be combined with insurance, other offers or discounts, and do not cover procedures completed by specialists outside our practice. Some exclusions apply to the 10% discount. Fluoride is not included in the adult or periodontal plans. Questions? Ask our front desk; we are happy to walk you through it.
